INSURED PATIENTS FAQ’S:
How much does it cost?
You will pay $35, $50, or $75 when you book the appointment which covers a deposit for the travel fee (non-refundable for no-shows and cancellations within 24 hours) and another $200 after you see the doctor. Typically, travel fee prices increase base on miles/time traveled whichever is greater. Price adjustments will be reflected on final billing and may increase or decrease if the patients selected deposit/travel distance was found to be greater than or less than the physicians travel distance/time.
0-25 miles/0-30 minutes = $35
26-50 miles/ 31-60 minutes = $50
51-75 miles/ 61-90 minutes = $75
Routine Foot Care Appointments
To ensure our doctor gets covered for services performed, we will have patients or their power of attorney (POA) sign an Advanced Beneficiary Notice (ABN) stating that they have not had routine foot care services (Nail trimmings/debridements and callus debridements) performed within the past 60 days by another qualifying physician who billed for these services. Medicare and CMS policy state that only one qualifying health care provider can render these services during the allotted timeframe so the ABN form acknowledges this and states the patient or POA will be held responsible for paying the service fees by our CFAS physician should any violation occur.
No Restrictions or Referrals Needed- applies to self pay patients and most major insurance plans
See the doctor of your choice, not your insurance company’s (excludes HMO plans, and Skilled nursing Facility residents)
How much does it cost? What to expect to pay day of service?
In addition to co-pay which varies amongst insurance plans, there is a travel fee for our CFAS specialist. You will pay $35, $50, or $75 when you book the appointment which covers a deposit for the travel fee (non-refundable for no-shows and cancellations within 24 hours).
(The travel fee is recurring and in addition to any co-pays for insured patients, and new patient/consultation or established visit/follow-up visit fees and procedures fees for self-pay visits, respectively.)
Do you accept Medicare?
Our CFAS physician is a Medicare provider. If the doctor is not a Medicare provider, they will not accept your appointment request as a Medicare patient. *Patient must have valid Medicare coverage that includes in-office consults (Part B), physician must be a Medicare provider, and the specialty type must be entitled by Medicare.
What insurance do you take?
The CFAS platform does accept insurance. A list can be found here https://comprehensivefootandanklespecialists.com/insurances-accepted/. Alternatively, your CFAS appointment can be paid for with your HSA, FSA or major credit card. If the doctor is in your insurance network, any subsequent consults, lab tests, procedures and follow-up appointments can be paid for by insurance.
Will this apply to my deductible?
No (most people never reach their deductibles). However, with CFAS, you can use your HSA (Health Savings Account) or FSA (Flexible Spending Account) or credit card.
What if I don’t have insurance?
No problem. There is no referral needed for the first appointment. Payment is made via HSA (Health Savings Account), FSA (Flexible Spending Account) or credit card.
What if I haven’t met my annual (yearly) deductible?
Sometimes, not going through insurance is a cost-saving method for patients without major medical illnesses on high deductible plans! If you haven’t met your health insurance deductible for the year, you’re an ideal candidate for CFAS. Since high insurance deductibles are rarely met, we welcome payment by HSA (Health savings Account), FSA (Flexible Spending Account) or credit card.
I need to use my HSA (Health Savings Account)
CFAS is one of the HSA Eligible Expenses. Simply input your Health Savings Account number and the cost of travel + $200 visit will be deducted without any work on your part.
What if I cancel my appointment?
As long as it is not within 24 hours of the appointment, you will receive a full refund.
What if I’m in college/I’m the parent of a college student?
Student healthcare is our priority too. CFAS accepts all major credit cards. Student health insurance can be used for subsequent visits.
I need to book a doctor visit, but hate waiting on hold.
Book a doctor appointment online on our website using the CFAS self service widget. Your appointment will occur with a nearby doctor in person. No phone call or waiting on hold necessary. Of course, phone call and email are always available options.
