SELF-PAY FAQ’S:
How much does it cost?
A flat fee of $250 for the consultation. You will pay $35, $50, or $75 when you book the appointment which covers a deposit for the travel fee (non-refundable for no-shows and cancellations within 24 hours) and another $200 after you see the doctor. Typically, travel fee prices increase base on miles/time traveled whichever is greater. Price adjustments will be reflected on final billing and may increase or decrease if the patients selected deposit/travel distance was found to be greater than or less than the physicians travel distance/time.
0-25 miles/0-30 minutes = $35
26-50 miles/ 31-60 minutes = $50
51-75 miles/ 61-90 minutes = $75
The $200 covers the doctor’s home visit, independent/assisted living facility visit, or memory care visit (think of this like an office visit charge), additional time required during initial encounter discussing and reviewing medical history and the history of present illness, obtaining any necessary vitals, obtaining and inputting patient HPI in our EHR, additional time required for initial physical exam, and covers the price of performing any basic routine foot care procedures such as trimming nails and calluses (additional fees may be included for high level procedures such as ingrown nail removals, biopsies, hammertoe correction/straightening, joint aspiration, incision & drainage, de-roofing of blisters, excision of soft tissue mass/lesions, steroid injections, gout removal, ulcer/wound debridements, and more). A full list of self-pay costs of common podiatric procedures and associated costs can be seen below. This price reflects only the initial consult visit. Subsequent visits/follow up visits for routine care prices decrease and reflected solely based on the procedure(s) performed on a fee for service model. Please see procedure lists and their associated costs below for expectations on established patient/follow-up visits.
What is included in the $250 cost?
Each appointment booked through CFAS is $250. This is for an in-office consultation or an online, telemedicine conference with a top doctor regarding the medical complaint that you list on the HIPAA-protected appointment request form while booking your appointment. All other consults, lab tests, procedures and follow-up appointments are considered a separate encounter, and paid for separately.

Will this apply to my deductible?
No (most people never reach their deductibles). However, with CFAS, you can use your HSA (Health Savings Account) or FSA (Flexible Spending Account).
What if I need surgery or additional testing?
CFAS is for the initial consultation only. The $250 guarantees you an in-office visit with a Fellowship Trained Board Qualified Podiatric Surgeon specialist. All subsequent care can be through your insurance. If you do not have insurance and need surgery, in person procedures can be performed at discounted rates as needed. Please contact our answering service to get a quote prior to booking.
What if I cancel my appointment?
As long as it is not within 24 hours of the appointment, you will receive a full refund.
What if I don’t have insurance?
No problem. There is no referral needed for the first appointment. Payment is made via HSA (Health Savings Account), FSA (Flexible Spending Account) or credit card.
What if I haven’t met my annual (yearly) deductible?
Sometimes, not going through insurance is a cost-saving method for patients without major medical illnesses on high deductible plans! If you haven’t met your health insurance deductible for the year, you’re an ideal candidate for CFAS. Since high insurance deductibles are rarely met, we welcome payment by HSA (Health savings Account), FSA (Flexible Spending Account) or credit card.
I need to use my HSA (Health Savings Account)
CFAS is one of the HSA Eligible Expenses. Simply input your Health Savings Account number and the $250 will be deducted without any work on your part.
Is CFAS an affordable health care option?
Considering that the average co-pay for an emergency room visit is $250, plus $50 co-pay for an office visit (usually with the Physician’s Assistant), plus $175 co-pay for a specialty care office visit, CFAS is an affordable health care option in that you pay one flat fee of $250 for one visit to a specialty care doctor who is trained to treat your specific medical concern.
