Terms and Conditions for Comprehensive Foot & Ankle Specialists

By scheduling an appointment, visiting our office, or using our services, you agree to comply with these Terms. If you do not agree with any part of these Terms, please do not use our services.
1. Medical Services
The practice provides podiatric medical and surgical services as outlined on our website and in our communications. All services are provided by licensed healthcare professionals, including podiatry and podiatric surgery services.
2. Patient Responsibilities
You are responsible for providing accurate and complete information regarding your medical history, current health status, and any other relevant information necessary for proper care.
You must notify us of any changes in your contact information, insurance details, or medical condition.
You are responsible for adhering to prescribed treatments, medications, and follow-up appointments.
3. Appointments
Appointments can be scheduled via phone, email, or through our online system.
We require a minimum of 24 hours’ notice for cancellations or rescheduling. Failure to cancel or reschedule within this time may result in a $50 cancellation fee.
If you miss an appointment without prior notice, we may charge a “no-show” fee of $50.
4. Insurance and Billing
We accept various insurance plans. It is your responsibility to verify that our office is in-network with your insurance provider.
You are responsible for understanding your insurance benefits, including coverage and co-payment obligations.
For non-insured patients, we offer self-pay options. Payment for services is due at the time of service, unless otherwise arranged.
If there is a balance remaining after your insurance has processed a claim, you will be billed for the remaining amount.

We accept payments via cash, credit card, or other approved methods.
We may charge late fees or interest for overdue payments.
